Loan Disbursement Allocation Request

Instructions:

This form allows you to authorize funds included in your Cost of Attendance (COA) to be used toward charges that are not eligible for federal financial aid.

Select Yes if you would like your available COA funds to be applied to these charges. Select No if you do not authorize this and understand that you will be responsible for paying the balance.

Your selection will remain in effect as long as you are enrolled in both programs, unless you contact the Financial Aid Office to change or revoke your authorization.
