Alumni Award Nomination Form

We invite you to nominate a graduate of Rocky Mountain University of Health Professions for one of the following awards;

Outstanding Alumni Award: This award is one of the highest honors a Rocky Mountain University of Health Professions (RMU) graduate can receive for distinguished contributions to the recipient’s profession and for service to the University or community at large.

Early Career Alumni Award: This award is presented to a graduate from any RMU program for their drive, passion, and commitment to their profession, advancing healthcare, and service to the University or community at large.

For each nomination you will need to submit the following;

Nominee and nominator(s) information
A nomination letter
Endorsement letter from a second individual (if self-nominated)
Nominee’s resume or curriculum vitae

Nominations and all supporting documents must be received by RMU’s Alumni Relations team no later than April 1 of the award year.
