Graduation Application

The Graduation Application is used to officially apply for degree conferral and confirm that you have met, or will meet, all requirements for your degree. Submitting this form initiates the review process conducted by the Registrar’s Office to verify your eligibility for degree conferral.  If you apply and then are found ineligible for degree conferral, you will need to submit a new Graduation Application in the semester your requirements will be met.

The Graduation Application deadline is listed on the University Calendar.

This form does NOT register you to attend the commencement ceremony.

Program Degree Completion Requirements

All Program Requirements
  • All coursework completed with a minimum cumulative GPA of 3.00
Additional SLPD/OTD Requirements
  • Bound capstone approved and received by the University (or confirmation that the bindery has received it)
  • Electronic copy of the capstone (PDF including signature pages) received by the University
Additional PhD Requirement
  • Qualifying exam passed
  • Practicum completed and approved (if applicable)
  • All dissertation requirements completed (committee formation, proposal development and defense, IRB approval, final defense, and IRB closure)
  • Bound dissertation approved and received by the University (or confirmation that the bindery has received it)
  • Electronic copy of the dissertation (PDF including signature pages) received by the University
